New Patient Policy

If you are interested in becoming a new patient at The Center you can call us during regular business hours. Often, we can schedule a New Patient appointment within 1-3 business days or even sooner depending on emergency situations. When you call, a member of our office will be glad to take down your information and schedule you with your desired provider. To do so, we will need the following information at the time of your call:
• Name
• Date of Birth
• Social Security Number
• Contact Number
• Address

We will also need the following Insurance information:
• Insurance Company Name
• Member ID or Subscriber Number
• Insurance Contact Number
•Group Number (if applicable)
• If the new patient is a dependent or spouse of the original policy holder, then we will need the policy holder’s name, social security number and date of birth as well.

*Please note: Without this information, we cannot verify your insurance benefits. This could delay the scheduling of your appointment. We cannot hold an appointment until we have verified your insurance coverage.

Once you are scheduled with a provider as a New Patient, we require the following on the day of your appointment:
• Arrive 15 minutes early
• Bring your insurance card
• Bring a current form of Identification
• Appropriate insurance co-payment

Payment Policy
- We do not accept checks for any new patient appointments. Only cash, Credit/ Debit cards and money orders are accepted at your first appointment.
- At the time of your appointment, if you are unable to pay your responsible amount due for services,
 We will be glad to reschedule your appointment.

Reschedule/Cancellation Policy
- We ask that if you find the need to cancel or reschedule your new patient appointment, please give the staff 24 hours notice before doing so. The Center carries a policy which states that  failure to give 24 hours notice of reschedule/cancellation for a new patient appointment could result in future inability to schedule with the practice again.

 

Insurance Policy

We gladly accept a wide range of Insurance Carriers to help assist in the payment of your visits. We also have set self-pay rates for those who do not carry Insurance Coverage at the moment.
Here are a few Insurance Carriers that we do take:
• Blue Cross/Blue Shield                  
• Tri Care
• Medicare
• Aetna 
• United                             
• Cigna
• Magellan
• Humana
• Staywell/ Wellcare

These are just a few basic Insurance Plans that we do take. Please call our office to check if we are
In-network with your specific Insurance Carrier.

If you do not have Insurance
- If you do not have insurance, the office can set a Self-Pay rate with you upon request, at the scheduling of your appointment. Please call for further information.

Change of Insurance
- If you have a change in insurance, please call the office at least 24 hours in advance of your next scheduled appointment. We will need to collect your new insurance information and verify your benefits.
*If you fail to do so, we will be happy to help you reschedule your appointment.

Patient responsibility
- Patients are responsible for any and all services not paid for by their insurance company. We collect your benefit information, request authorizations and bill to your insurance as a courtesy. If for any reason there is a problem with coverage, it will then become the patient’s responsibility and will be billed accordingly.
